Your organization must be non-profit; therefore we will need your Federal Tax ID Number.

-       We must receive all information prior to starting your fundraiser.
-       One master order form should be sent to Natural Candle Pastries Co.  Please ask each                                  participant to retain a copy of their order form to distribute his or her orders.
-       Payment must be sent with master order form before shipping will occur.  Allow four weeks for
        shipping from the date that we receive your order.
